The evaluation listed below is for a comprehensive evaluation of a child who is 5 years old or older. Please call for a price quote.

Your child’s evaluation will include the following:

• Initial parent interview
• Teacher interviews
• Data gathering from other professionals (counselors, related service providers, physicians, tutors)
• School visit/classroom observation
• Comprehensive testing with your child
• Scoring of the tests
• Writing of the report
• Parent follow up meeting
• Follow up contact with the referring professional

I discuss all billing prior to providing services to avoid confusion or miscommunication.

Payments: Parents are asked to submit the evaluation fee in full at the first session. Payments can be made in cash, check, Venmo or Zelle.

Insurance: I do not participate in any health insurance programs. However, after you have pre-paid the evaluation fee, I can provide you with an itemized bill which supplies the procedure and diagnostic codes that health insurance companies need in order to reimburse you.
